Reflections Community Support is a private Domiciliary care provider based in Guiseley, Leeds. We provide a range of care and support services delivering bespoke packages from personal one-to-one care, social calls, short/long respite, dementia care, stoma and peg feed, and development of independence.

We also specialize in providing care and support for individuals with learning difficulties, challenging behaviour, mental health, and assistance with meals and if necessary sleep-in, waking night and live-in carers.

We’re seeking to recruit multi-skilled Team Leaders & Support Workers with the right values and attitude to work with us at our Domiciliary care organisation.

  • Must have a Valid UK Driving License and have access to own vehicle.
  • Must have successfully obtained an NVQ Level 2 in Health and Social Care.
  • Must be comfortable in working shifts across the following areas; LS19, LS20, LS21 and LS28.

E-Rota System – Shift Pattern:

  • Early shift (07:00 –14:30)
  • Late shift (16:00 – 22:00)


This will include and not limited to;

  • To provide the highest quality care to service users in their own homes, in accordance with their support plan, via the Access Care Planning App.
  • Community Support Worker will be required to promote independence and assist where necessary with personal care, domestic/practical tasks, and enabling.
  • Personal care includes washing, dressing, bathing, toileting and administering medication once trained.
  • Social duties include preparing and/or cooking meals, essential cleaning, shopping, assistance with bill payments and fire maintenance.
  • Enabling and support may include such activities as supervising, monitoring and coaching to empower service users to be as independent as possible and have control over their own lives. Support Workers may also be required to escort or accompany service users to outings, appointments or other events.
  • To be aware of the personal needs and requirements of your service users, communicating clearly and effectively is of crucial importance not only with the service user but with their family, friends and other healthcare professionals.
  • To ensure that all care services are completed in line with the Reflections Community Support Policies and Procedures, Guidelines and Frameworks including the CQC Fundamental Standards and all contractual and legislative requirements.
  • Any other duties that may be necessary to comply with the demands of the CQC or other legislative bodies.
  • Maintain appropriate records to meet the needs of the service, observing and promptly reporting back any changes or concerns in your service user’s circumstances or condition.
  • Contribute to and work effectively as part of a team.
  • Deliver support in a manner which is sensitive to the service user’s culture, disability, race, age, sexuality, marital or civil partnership status, transgender status or religion/belief.
  • Engage and work closely with unpaid carers, such as family or friends, for the benefit of the service user.
  • Promote anti-discriminatory practice.
  • Protect the confidentiality of service users and of Reflections Community Support.
  • Promote the dignity and privacy of service users.
  • Act promptly and appropriately to protect service users from neglect or abuse.
  • Participate in staff development (including supervision), training and performance appraisals as required.
  • At all times present a professional and caring image of Reflections Community Support and demonstrate behaviours reflective of Reflections Community Support values.

To support the Registered Manager in the day-to-day running of Reflections Community Support (RCS).


  • Minimum of one year experience of working as a Team Leader/Coordinator.
  • Must have obtained an NVQ Level 2 in Health and Social Care.
  • Must hold a valid UK Driving License.
  • Must have access to own vehicle.
  • Must be able to work alternate weekends
  • Must be prepared to do on-call (full training will be provided)

E-Rota System – Shift Pattern:

  • Early shift (07:00 –14:30)
  • Late shift (14:00 – 22:00)


This will include and not limited to;

  • Provide leadership to the team acting as a positive role model at all times.
  • Lead the shift, ensuring staff give support that follow the individual’s care and support plan.
  • Provide effective supervision minimum of two staff members a week, spot checks to be conducted which includes weekly medication audits during your shift.
  • Provide on call cover as arranged by the Registered Manager, working within on call guidelines.
  • To create or review Care and Support Plans, Risk Assessments, Service User Tasks/Activities.  
  • One day a week will be stationed at the main office in Guiseley to update electronic systems.
  • Covering, holiday and sickness.
  • Liaising with multi-disciplinary teams.
  • Supporting inducting and mentoring new staff.
  • Listen, support and resolve service users’ queries or questions as required.
  • Allocation of daily calls for new service users or holiday/sickness cover as required.
  • Has an eye for detail, motivate, support and mentor the staff where/when needed.
  • Compiling weekly Rota’s (Reconciliation)
  • Proficient at using Access Care Planning and People Planner (A definite advantage)
  • Good understanding and use of I.T. Skills, Microsoft Office and able to work with bespoke packages.
  • At the end of each shift to check and ensure staff have completed all necessary documentation and completed assigned tasks via Access Care Planning.
  • Enter actively into supervision and appraisal meetings.
  • Attend training on a regular basis to ensure that all mandatory and personal training and development needs are met in line with company requirements.
  • To work in accordance with the company’s mobile phone and IT policies.
  • To attend both Team Leaders and Staff Team meetings and on a rotation basis you will be required to take minutes and type up using the correct format as directed by the Registered Manager.
  • Answer the calls in a professional and courteous way and to ensure all messages are delivered to the correct individual.

Administrative Duties

You will be required to work flexibly over five days out of seven on a rota basis to meet the needs of the service, this includes weekends, bank holidays, and sleep-ins if required. You will also be responsible to work one day a week from the main office in Guiseley where you will be answering phones, taking notes, updating service users Care & Support plans, activity tasks, and assisting the Registered Manager with any assigned duties. Ensure work is recorded accurately and appropriately and in compliance with the organizational requirements and that records made and any personal information used is in compliance with the GDPR Policy and any guidelines or standards issued by the Government or the Care Quality Commission (CQC).
